Personal Health Data Workbook

This Health data workbook uses a 2 step (summary-detail) structure so you (and your physician) can see a summary (and trends) of your full health care history as well as dig into the details using page links.

BONUS section inside book – “Suspected Allergies Isolation template” in Appendix – if you have suspected allergies to something in your lifestyle but have not been able to verify what is causing it – you will love this template to track and isolate the cause of the allergy.

As soon as you set up a new healthcare appointment, you can go to a “detail” page in this workbook and start writing out information that is important to your upcoming doctor visit (or procedure). During the visit, you should take this workbook with you to the doctor’s office, so you can remember to share all the details for a great decision from the doctor. After the visit, you can note in the detail pages relevant information for next steps as well as link the details page over to a summary page / future visit page.

This workbook allows us to be in control by writing all data from multiple sources in one location. Electronic systems use database technologies like relational databases and master-detail concepts to efficiently bring data together. We have applied the same concepts, but to a printed version so that you can see all your data in summary as well as details in this workbook.

Whether you are computer savvy or not, this workbook is a great way to use summary-detail linking technology to see summary of overall health along with details written on a separate page but linked together. 

We recommend every member in the house should have a workbook of their own, and a gift to parents / grandparents will make their life much easier to manage with respect to healthcare visits, imaging, test results management and Rx.
